Youth Camp Health Exam Record
  1. •Must be completed by a medical practitioner.
  2. •Any physical completed within 3 years of the start of camp is acceptable.
 
Administration of Medication Form
  1. •Please use one form for each medication.
  2. •Please be sure to fill out both sides.
  3. -Note: The chart on the second page will be filled out at camp, only fill out the top.
  4. •These forms are needed for all types of medicine, including inhalers, allergy medicines, and OTC pain relievers (Advil, Tylenol, Motrin, Aleve, etc.)
  5. •All medications must be delivered to camp personnel in their original bottle, labeled with the campers name, name of medication, and directions for proper administration.
